Eligibility Guidelines

 
Group Plans
 

        To be eligible for coverage a company must:
    • Be actively in business
    • Enroll 1-50 eligible employees (owners included)
    • Be located in the Minuteman Health service area
    • Be a member in good standing of HSA

    ELIGIBLE EMPLOYEES

        An eligible employee is one who:
    • Works at least 15 hours per week for the employer
    • Was hired for a period of at least four months, i.e. not seasonal or part time
    • Resides or actively works in the Minuteman Health service area

    NEW EMPLOYEES

         All future eligible employees can be enrolled within 30 days of hire or within a time frame consistent
         with company new hire probation policy


    WAIVER OF COVERAGE

        Employees can be excluded who are:
    • Covered by a spouse’s or parent’s health plan
    • Age 65 or over and covered by Medicare or Veteran’s plan
    • Employees covered through Healthcare.gov
    • Living outside the Minuteman Health service area
    • For groups of 6 or more employees; if an employer has another sponsored health plan no waivers are required.  Employer must submit a copy of the other carrier’s most recent bill with a list of employees on the plan.

    PARTICIPATION REQUIREMENTS
       Groups of 2-50

    • 75% of eligible employees must enroll

      Groups that offer 2 carriers

    • At least 37.5% eligible employees must enroll

    PLAN SELECTION
    2 plans can be offered side by side down to 2 eligible employees

    EFFECTIVE DATES
    All coverage will be effective on the 1st day of the month, contingent upon the carrier’s enrollment requirements. Enrollment materials should be received by the 25th day of the preceding month.
     

    Individual Catastrophic Plan

    MyDoc HMO Simple Care meets the federal definition of a “Catastrophic Plan” and as such is only available to certain eligible individuals. You are eligible to enroll in this plan if you and each of your benefits eligible dependents:

    • Are under age 30 prior to the date of enrollment, -or-
    • Have received a certification from the Massachusetts Health Connector that you are exempt from the federal requirement to buy health insurance, also known as the Individual Mandate
