Requirements For A New Agency

Start the agency onboarding process if you are a new broker agency and intend to add multiple brokers to your agency account. The agency’s authorized contact will have a login to view the agency’s active book of business while brokers can only view and manage their own book of business. HSA Insurance will pay any commissions to the agency. We need the following documentation to complete your agency appointment:
  • Agency Setup Form
  • W-9 Form
  • Signed HSA Agreement
  • ACH Form
  • Proof of Errors & Omissions (E&O) coverage for the agency
  • Massachusetts business entity license as proof the agency is licensed to sell in Massachusetts
  • A Broker Setup Form for brokers that should get setup under the agency
  • Massachusetts producer license for each broker
  • Proof of Errors & Omissions (E&O) coverage for each broker

Start Process For New Broker Under Existing Agency

Start the new broker onboarding process for existing agencies if you need to be setup under an already existing agency account with HSA insurance. We need the following documentation to complete your agency appointment:

  • A Broker Setup Form
  • Massachusetts producer license for the new broker
  • Proof of Errors & Omissions (E&O) coverage for the new broker

Start Process For Independent Broker

Start the onboarding process for new brokers if you are an independent producer seeking appointment under your SSN. HSA Insurance will remit commissions directly to you, or you have the option to designate payment to a corporate account. We need the
following documentation to complete your broker appointment:

  • Broker Setup Form
  • W-9 Form
  • Signed HSA Agreement
  • ACH Form
  • Proof of Errors & Omissions (E&O) coverage
  • Massachusetts producer license
